Pianoteq and Organteq have extensive and flexible MIDI-mapping capability. For those looking for a MIDI CC (continuous controller) hardware fader-based controller to use with MIDI mapping, these might be nice.

Studiologic SL Mixface looks more interesting (and less expensive)

9 assignable Faders, 8 assignable rotary knobs and 8 assignable buttons
...and with 4 different banks to switch if you need it
