Realphones has what they call "System-Wide Audio Processing".

"In order to process audio throughout your system, it is necessary:
. Launch Realphones System-Wide
. Select your output device in the settings of application"
I assume then that it if you select the same audio device in Pianoteq, it should work.
BTW its probably advisable to try the free trial of Realphones.

This is what I understood from your question: you would like to use Realphones with the Standalone version of Pianoteq.
Is it what you want to do?
If no, then my bad but if yes, then I pointed you to the solution on how to do this.
What I falsely wrote was that I assumed one has to simply choose the same output device on both applications.
But it is a little bit different and this is BTW quite clearly explained in the link I provided.
In the meantime I tried it and it works on my system though it adds some latency.
1. You have to select the audio device connected with your headphone in Realphones first
2. Select Realphones System-Wide as the sound output on your OS (for example "Line (Realphones System-Wide)" in Windows 10)
3. In the audio settings of Pianoteq you have then to choose
Audio device Type: Windows Audio
Audio output: Line (Realphones System-Wide)
