I've been experiencing missing notes recently, I believe since I upgraded to Mavericks (although there's a chance my memory may be unreliable).
I don't think there's anything wrong with my keyboard; built in synth seems ok. I haven't yet got around to testing with any other software, so I should make that a priority. My system is a 2011 27" iMac with an Edirol FA-66.
I've been having problems with Maverics, Pianoteq 4.5.3 and Mainstage 3.0.1. Pianoteq loads CPU > 100% (meaning only pianoteq plugin running on mainstage) and there are audio dropouts all the time, pianoteq goes mute all the time for a second or two. I can't say where the problem is, it might be in the audio interface (focusrite saffire) drivers, maverics, mainstage or pianoteq. But pianoteq is the only instrument that's doing this. When playing a patch with some other plugins as well, the plugins on the other channels keep on playing correctly while pianoteq mutes. But still I'm not saying pianoteq is the troublemaker here, might be that mainstage/audio interface drivers has problems with maverics and it's causing the CPU overload and pianoteq suffers from that.
As expected, my dropped MIDI notes do not seem to be a Pianoteq issue. Still would like to know if anyone else has experienced similar issues.
I haven't noticed any change in the number of dropouts etc since upgrading to Mavericks. I do get a few (sometimes going mute for a second or so as well), but again, I haven't done much to determine whether this is specific to Pianoteq or how different that would be if I took more care to close other programs, change buffer sizes etc.
The main other sound software I use at the moment is SuperCollider, but I haven't really done much audio on this mac apart from playing Pianoteq standalone.

Have you tried some other software instruments, and do they experience missing notes too? You could try monitoring incoming MIDI data with some utility and see if your keyboard sends all note on/offs.
This might sound a bit stupid, but: Could Pianoteq be running in demo mode? In demo mode some keys do not produce sound, so these might be the "missed notes".

I have tried briefly with GarageBand and seem to be dropping notes there as well, which is why I say it doesn't seem to be a Pianoteq issue. I'd still be interested to hear whether or not anyone else has experienced similar issues since upgrading to Mavericks though.
I'm not running in demo mode, but it doesn't do any harm clarifying these things sometimes.
Upgraded to OSX Mavericks this weekend. While playing last night, every so often the sound would drop out, particularly during complicated parts -- as if the polyphony had gotten to be too much for the CPU to handle. I'll have to try the 4.5.3 patch and see if that makes any difference...
Updated to 4.5.4 and that seems to have fixed the problems (so far!). Thanks for the quick patch.
